Office Policies

  • Office visits: New Patients and patients who have not seen Dr. Littles in the last year must arrive at least 30 minutes before the scheduled appointment time. This will allow for adequate time to complete/update any required forms. Any patient who is late for their appointment may be asked to reschedule.

  • Cancellation policy: As a courtesy, the office attempts to remind patients of their appointment ahead of time. However, it is the patient’s responsibility to remember their scheduled appointment. We ask that patients please if they are going to miss an appointment so that the slot may be opened for another patient.

  • All charges for services rendered in this office are ultimately the patient’s responsibility.

  • Co-Payments: Co-payments and deductibles must be paid at check in or check out. If your co-payment or deductible amount is not indicated on the insurance card, you may be asked to pay 20% of the total charge for the visit during check-out. If there is a credit on your account after all insurance payments have been received, a refund will be issued to you. Cash, checks, and all major credit/debit cards are accepted.

  • If we participate (are contracted) with an insurance plan under which you are covered, we will bill the insurance carrier for you. You will be responsible for any remaining balance, including annual deductibles, co-payments, coinsurances, and charges for non-covered services.

  • Insurance re-billing charge: Patients must provide our office with correct and current insurance information. If an insurance claim requires a second submission because of incorrect or outdated insurance policy information, there may be an additional $25.00 charge that is billed directly to the patient. If the correct insurance information is not obtained before the insurance company’s filing deadline, then the patient will be responsible for the entire cost of the visit.

  • Refill policy: Patients should ensure that they get all medications refilled during their visits. Should a refill be necessary between schedule visits, the patient should call the office and select the menu option for prescription refills. Be sure to include your name, contact number and medication name/dosing information in the message in order for your refill to be processed.

  • There is a $30.00 processing fee for returned checks. All refill requests are processed on Fridays. Patients should make every effort to ensure that they do not run out of medications, but should this occur and a refill be necessary before Friday, a $6.00 fee will be charged. Refills are granted at the discretion of Dr. Littles and may be denied if the patient has not been seen in the office in the last 6 months.

  • Dr. Littles reserves that right to terminate patients from the practice. Reasons for termination may include, but are not limited to, disruptive and abusive behavior, non-payment, failure to follow clinical advice or treatment, and failure to keep follow up appointments.
